PARENTAL CONSENT FORM

As parent/ guardian I hereby give my consent to my son/daughter attending/participating in the various activities of Knockconny Baptist youth /children’s work as outlined in the programme distributed at the start of the season.I also consent to photographs of my son/daughter being taken during activities and understand that these may be displayed at church events or published in the local press, for example, following charity fund-raising events. This consent is given on the understanding that any photographs taken will be used in a manner consistent with the church’s child protection policy. Transport arrangements (i.e, by coach, car or minibus) will be announced for each activity.Receipt of a completed consent form and your son/daughter’s attendance, will be taken as consent to participation in the relevant activity.
